Our primary goal is to provide quality medical care by using evidence-based care that is also easily accessible and responsive to you in your time of need.

We Are A Patient Centered Medical Home

As a Patient Centered Medical Home, our approach is to provide our patients with comprehensive health care, which is focused on all aspects of your health and overall well-being, including emotional, family and social concerns. Along with your physician and other health care providers, you are the most important person in managing your health.

A “Medical Home” makes it easier and more comfortable for you to access care on a day to day basis by strengthening your relationship with your primary care provider and the team responsible for your care. With a medical home, your quality of care will be significantly improved, and it will take less time for you to get the care when you need it.

How You Can Help

  • Talk with your primary care provider and team about any questions you have.
  • Keep in touch with your team if further questions arise about your health.
  • Take care of your health by following the plan recommended by your team.
  • Schedule a complete physical exam at least once a year.
  • Always let us know how we’re doing and how we can improve.

Our office is open Monday-Thursday from 9 a.m. to 5 p.m., Friday 8 a.m. to 5 p.m., with extended hours from 7-9 a.m. and 5-7 p.m. Monday-Thursday. Saturdays we are open for sick patients only. Every effort is made to see our patients for medical problems during daytime hours by calling 904-388-4646. Please note that our schedulers are available every day and will do their best to accommodate you. Our office also has a triage line {904-388-4646 x 22} during the day to answer any questions you may have. Booking an appointment is essential to ensuring all patients receive the time they require for quality medical care. After hours care will be provided by the on-call physician, who can be reached by calling our office directly at 904-388-4646. Need Tylenol/Motrin dosage instructions, or medication refills, or just want to send a non-urgent message try visiting our patient portal, which can be accessed from our website jaxpeds.com. In addition, our office provides a number of services like ADD/ADHD, Anxiety, Depression Management, Suture/Staple Removal, Strep, Flu, RSV Testing, Same day appointments and Sports Physicals for the convenience of our patients.

What You Can Expect

As your Medical Home we want to ensure that you understand what is expected of you and what you can expect from us. To that end we hope that you will take the time to review the following information and ask any questions that you have regarding our Medical Home model of care. Below is an abbreviated list of some of the things you can expect and that we expect from you as a part of the Medical Home model.

What we expect of you:

  • Communication regarding any changes to your clinical information
  • Communication regarding any changes to your demographic information
  • Notification from you regarding any visits that your child has had outside of our office
  • Ask questions about anything that you do not understand
  • Contact our office before going to the ER unless it is a life-threatening emergency

As a Medical Home we have a clinical lead and a Medical Home manager who lead our team to ensure that we are the best Medical Home that we can be. Below is a little information about each of these representatives. However, should you have questions about our Medical Home model please feel free to speak to anyone on our team.

  • Dr. Ryan Cantville : Clinician Lead – Coordinate and oversees the activities of the clinic and clinical team members to ensure compassionate, effective and efficient delivery of high-quality clinical care for patients.
  • Jana’y Raines, Office Manager : PCMH Manager – Care managers are responsible to initiate, facilitate, and monitor specific patient activities, interventions and chronic care protocols that will become the patient’s care plan.

  • Medical Records – In the event you are in need of Medical Records please call 904-388-4646 x 11. The Medical Records process takes, on average, a week.
  • After-hours – Jacksonville Pediatrics is available 24/7 365 days a year. If you are needing to get in contact with us after-hours please call 904-388-4646, you will be connected to our after-hours call service. The answering service will page the on-call Dr, and you will receive a call back within 30 mins.
